The Analysis with Aortic Stiffness, Carotid Intima-media thickness and Aortic Valve Sclerosis according to Existence and Severity of Coronary Artery Disease in Hypertensive Patients

Background : Indices of arterial structure and stiffness are proposed as surrogate markers of cardiovascular disease, but there were few studies examined multiple markers in the same population. We examined the carotid intima-media thickness(IMT), aortic valve sclerosis(AVS) and indices of arterial stiffness, including pulse wave velocity(PWV) and augmentation index(AIx) in hypertensive patients with suspected coronary artery disease(CAD).
Method : The present study included 57 hypertensive patients with suspected coronary artery disease and who were scheduled to undergo coronary angiography(35 males, 22 females, mean age 64.1¡¾9.3). IMT and AVS measurements were done using high resolution ultrasound machine. PWV was measured using PP-1000(Hanbyul meditech Co., Jeonju, Korea) on supine position. Three PWV including aorta, leg and arm were obtained. AIx of central pressure wave was measured using GAON 21A(Hanbyul meditech Co., Jeonju, Korea) at the radial artery.
Results : Indices of aortic stiffness(aortic PWV p=0.010, central AIx p=0.023) and carotid IMT(mean IMT p=0.007, max IMT p=0.009) were significantly higher in hypertensive patients with coronary artery disease. There were statistically significant associations between the severity of CAD and mean IMT(p=0.001), and max IMT(p=0.001). Of the parameters, the aortic PWV was independently associated with coronary artery disease in hypertensive patients.
Conclusion : Aortic PWV is useful in predicting coronary artery disease in hypertensive patients.
